Editorial: Nepal's postgraduate medical education faces significant structural and resource challenges. The National Academy of Medical Sciences (NAMS) has been at the forefront of training specialists since 1982, yet systemic issues—inadequate faculty, poor infrastructure in provinces, brain drain, and lack of standardized assessment—continue to hamper quality. As Rector, the author outlines the "Expanded NAMS" vision: establishing provincial campuses, standardizing curricula, improving faculty retention through better compensation, and integrating research culture. The path forward requires coordinated efforts from government, medical institutions, and the profession itself.
